Initial Consultation

Service Description

In your initial appointment we cover your full case history to gain better understanding of your state of health, help to address the root cause of your ongoing symptoms, establish a baseline, and set some goals. Occasionally, follow up lab work may be required and will be discussed here. What is included: • Comprehensive review of Your full health history including: presenting symptoms, whole body system review, past medical history, family health history, diet, and lifestyle. • Reviewing questionnaire, you will fill before the appointment including toxicity questions, current medications. • Reviewing your food diary. • Reviewing supplements you take. • Reviewing existing laboratory tests. • Determining missing pieces of your health puzzle. • Determining the best approach for your case. • Written plan including herbs, supplements, and diet recommendations. What is not included: • Medical tests. I ask that prior to your first appointment, you take the time to complete our initial questionnaire which will be send to you by email. I also ask that any recent blood work, imaging, or testing be sent to us in advance of your visit in order to more efficiently use our time together. Within 48-72 business hours, a plan will be emailed to you outlining my recommendations. These often include herbal prescription, functional testing suggestions, supplements, diet, and lifestyle recommendations.

Cancellation Policy

My goal is to provide quality health care to all clients in a timely manner. Please understand that appointment times are limited. If you need to cancel your appointment, please send an email or call. Cancellations has to be done minimum 24 hours before the appointment. If appointment is not cancelled in time, charge will be 50% of the service fee.
